I was wondering the same as Eric! I presume you are meant to use a fresh set of users at each stage?

This is probably a stupid question but I don't think it was specifically addressed: so at each testing iteration, do you use those same 5 or 6 people? Or do you use a new set of people?

Not every specialist agrees with your focus on asking "why?" type questions. Take a look at:
Why you shouldn't ask “Why?” in a usability test

hiya,
Another interesting post there paul, but i would stay away form your post bag for a few weeks just to be safe (remember, if it ticks or its in a metal envelope, chances are its bad) you know what there web types are like then they are angry....
Anyway, on a more serious point, i think this is an interesting method, i have a project which i think this may be a very good idea to do this with.

Hi - I'd like to point people to a new service that I used recently. The company is UserTesting.com (www.usertesting.com) and they use a similar approach to the one you describe above. I tried it on a project and was impressed with the video I got back from each of the testers. It really helped my client and I see their website in a new way.
I hope this is ok to submit a comment that is essentially a plug for another company, but UserTesting's service is so similar to what you wrote about, I couldn't resist.
